EVALUATION AND TREATMENT  FEES

Fees are based upon # of problems and complexity, which is approximated in the following table.

NEW PATIENT FIRST 30 MINUTES $100 EVERY 10 MINUTES THEREAFTER $30
ESTABLISHED PATIENT FIRST 20 MINUTES $50 EVERY 10 MINUTES THEREAFTER $15

Compare with standard automobile mechanic engine diagnostic of $90.  You will be present with us doing work for you.  Remember your most valuable asset... YOU!

 

IMMIGRATION EXAMINATION (I-693 FORM) FEES

AVIATION MEDICAL EXAMINATION AND BASIC MED FEES FOR 2ND AND 3RD CLASS PILOTS

VACCINATION + TESTING FEES

Blood draws, non-routine vaccines, and forms are seldom paid for by health insurances.  Payment will be requested prior to these services.  Payments subsequently paid for by the insurance company of <$15 will be credited back to your account (unless otherwise requested) and >=$15 will be fully refunded back to you.  Most health plans will pay for routine vaccinations and laboratory testing.

If these fees are not affordable, Downtown Family Health Center is a medical clinic (partially subsidized with government funding) which offers lower cost or free care to all, including the homeless.  You may call 619.515.2525 for details.

POLICIES

ELIGIBILITY IS NOT A VERIFICATION OF COVERAGE

Health insurances may not fully cover all services such as blood draws and non-routine vaccinations.  We offer these to our patients and bill their health insurance as a courtesy.  Please check with your health plan regarding Responsibilities (accessible on their website) including co-payments, co-insurance, and deductibles for Sick Visit, Wellness Visit (check up), and Tests.  Most of our services are covered by most insurance plans.  With recent health care reform, preventive services are increasingly covered.

VERIFICATION OF IDENTITY + INSURANCE

Please bring current PHOTO GOVERNMENT IDENTIFICATION (passport if you are here for Immigration Examination) and HEALTH INSURANCE CARD.

If you are not prepared with this, payment in full is required prior to the visit.  Incomplete, inaccurate, or late insurance information will be charged Late Information processing fees; if proper information is received:>1 month $40, 1-4 weeks $30, <1 week $20, <24 hours $10.

Please check-in here 15 minutes prior to your first appointment.  You are responsible for updating us with any changes in address, phone #, or insurance.

Immigration Examination patients, please bring your current passport or employment authorization cards and any previous (childhood) vaccination records if you have them; if you don't have them it is alright.  You do not need to bring the I-693 form because we have an electronic form.

CHANGE OF INSURANCE OR PCP WITH MANAGED CARE HEALTH PLANS

Managed Care health plans such as HMOs require that you designate a Primary Care Provider (PCP) in an affiliated medical group.  Our PCP is Sunita Shailam in Mercy Physicians Medical Group (MPMG).  If you have had a previous PCP, call your insurance to switch back to that person to preserve your continuity of care.  If you would like to see us and meet Qualifications listed here, contact your health plan and this will be effective the first of the NEXT 1-2 months and may take another month to show up on the MPMG website.  

If you are not yet active on the Mercy Physicians Medical Group website and are assigned to us, in order to avoid problems with payment with us these are your options:

     1.  request PCP change | preferred if you have had a previous PCP in town switch back to that person to preserve continuity, call your health plan (Aetna / Anthem Blue Cross / Blue Shield / HealthNet / United Healthcare)

          OR

     2.  request website activation | call Mercy Physicians Medical Group 619.543.8800 Customer Service x0, call us back later to confirm whether you are activated

For our established patients, every time you change insurance, verify your PCP as soon as you receive your insurance card so you will have access when you need us.

PPO plans are NOT managed care health plans and do not need to officially designate a PCP to be treated here.

PAYMENTS

DUE DATE

Balances, co-payments, and deductibles are due at the time of the visit.  If your health plan reimburses us for these services, you will be offered a refund.

METHODS

Cash, Master Card, Visa, and Discover are accepted.

Bring current health insurance information and government-issued photo identification to each appointment for your identity protection.

We follow The Payment Card Industry Data Security Standard (PCI DSS) for your financial safety.  You may pay at the Point of Service POS here with our terminal and enter your card yourself.  We do not store any financial information.
